Vaccine Exemptions in Indiana

School immunization requirements are determined by the Indiana Department of Health. Indiana has both medical and religious exemptions to vaccinations.

According to Indiana Code 20-34-3-2, a religious exemption requires a written statement from the parent(s) stating they object to vaccination based on religious beliefs, signed and dated and sent to the appropriate school personnel (usually the school nurse). You are NOT required to explain why your religious beliefs go against vaccination. You can find a sample letter here.

Medical exemptions are allowed and need to be written by a licensed Indiana physician (IC 20-34-3-3).
